Coordination of Community Services

The Queen Anne’s County Department of Health helps children and adults with developmental disabilities get the care and support they need. Our coordinators work directly with individuals and their families to see if they qualify for Developmental Disabilities Administration (DDA) funded programs. Once enrolled, we provide ongoing support tailored to each person's unique needs.

Connecting You to Care: We guide you toward community, state, and federal programs so you can easily access the right services and resources.

Making Friends and Community Connections: We work with you to create opportunities to build meaningful relationships, make new friends, and feel at home in your community.

Guiding Your Programs and Plans: If you use a DDA waiver program, we help keep your enrollment active and make sure your daily services match your Person-Centered Plan. Most importantly, we regularly check in with you to make sure you are happy with the care you receive.
